As the health plan with a heart, it is our mission is to build healthier communities.

To do that, we created heart,
which stands for Health, Education, Access, Resources, and Tools.

Together, these form the basis for a happy, healthy life and we bring these to you through the heart Community Resource Center!

Here’s what you’ll find at heart:

  • Support groups to connect with others

  • Job support for resumes and interviews

  • Food, exercise and financial classes for all skill levels

  • and more!

Heart events and resources are free!
You do not need to be a Community Care Plan member.

Hope Florida Hope Line

Our goal is to bring together resources in our community to support you. When you call
1-833-GET-HOPE, you will talk to a caring Hope Navigator. They are ready to listen and help you with life’s challenges. They can help you in many areas of life so you can be successful.

Hope Florida helps many people in our community like:

  • Single parents,
  • People getting help from the government,
  • Young people who are aging out of foster care,
  • People with unique skills and their families,
  • Seniors,
  • Juvenile justice youth,
  • Veterans,
  • And many others.
