Community Care Plan (CCP), the health plan with a heart, is pleased to announce that we have been awarded a contract to provide subsidized Children’s Health Insurance Program (CHIP) and full-pay health insurance coverage through the Florida Healthy Kids plan, which serves school-aged children from age five through the end of age 18.
Beginning January 1, 2020, CCP will provide coverage in the following eight Southeast and South Florida counties:
Introduced in 2000, originally as South Florida Community Care Network, as the first safety-net, hospital-owned Provider Service Network (PSN) in Florida, CCP currently serves members in Medicaid, uninsured programs, and self-insured employee health plans. Headquartered in Sunrise, Florida, CCP is owned by Broward Health and Memorial Healthcare System.
To provide the best quality of care to our members, including future potential members, our Provider Operations Representatives will be contacting providers in the next few weeks to discuss CCP’s contracting process for the Florida Healthy Kids program.
