    Community Care Plan and Papa Collaborate to Provide Social Support for At-Risk Medicaid Members

    Community Care Plan Aims to Close Gare Gaps and Improve Social Determinants of Health for Vulnerable Medicaid Members

    Today, Community Care Plan announced its collaboration with Papa, a curated platform of companionship and support for older adults, families, and other vulnerable populations. Community Care Plan now provides its qualified Medicaid members with Papa’s services, including companionship and help with light house tasks, kids playtime or completing homework, technology education, transportation, and other general assistance needs.

    In support of its mission to promote healthier communities, Community Care Plan continues its focus to improve the social determinants of health (SDoH) for its community and members. This includes removing barriers that affect one's health and improving human connection. A 2018 study from the American Cancer Society, "found that lack of social connection may be associated with obesity, heart disease, and smoking" . By working with Papa, Community Care Plan provides its qualified Medicaid members with the social support services that they may be lacking. 
    Using the power of connectedness to improve health and close gaps in care, Papa’s on-demand workforce of compassionate Papa Pals will address the specific needs of Community Care Plan’s Medicaid members. Community Care Plan will provide Papa Pals to Medicaid members ages 12 to 17 who have not had a well visit in the last calendar year, members who are pregnant or have delivered in the last year, and members who are age 55 or older. 

    “To improve the health of our members, we want to help improve human connection and remove barriers that affect their everyday lives," said Miguel Venereo, MD, Chief Medical Officer at Community Care Plan. “Whether it’s to talk to someone, go along to doctor’s appointments, watch a mom’s kids at the doctor’s office so she can focus on her appointment, or give that mom a break and help make sure that the kids get their homework done, Papa Pals are there to provide our members with support."

    Papa Pals are trained companions who provide human help for critical day-to-day tasks. By serving as eyes and ears in the home, these trusted companions are able to understand the needs of members and to connect members to plan benefits or vital community resources.

    Community Care Plan Works with Virta Health to Reduce the Growing Type 2 Diabetes Epidemic

    Community Care Plan announced a collaboration with diabetes reversal leader Virta Health. Virta’s virtual type 2 diabetes reversal treatment is now available to thousands of CCP’s eligible employees and adult Medicaid members.

    Florida has the 17th highest rate of type 2 diabetes in the country. Per the American Diabetes Association, nearly 12% of the adult population—almost 2 million Floridians—suffer from diabetes. Another nearly 6 million have prediabetes. Yet the burden is not shared equally; diabetes disproportionately affects lower-income adults including those covered by Medicaid plans like CCP.

    “Working with Virta is a major step toward reversing South Florida’s diabetes epidemic,” said Miguel Venereo, MD, Chief Medical Officer at Community Care Plan. “Though many digital health solutions aren't well-suited to Medicaid populations, Virta's personalized, high-touch treatment makes it easy for patients from any background to improve their metabolic health.”

    In Virta’s peer-reviewed clinical outcomes, the majority of completing patients achieved blood sugar below the diabetes threshold at one year, while removing or reducing the need for diabetes medications such as insulin. Patients also saw 12 percent weight loss and improved over 20 markers of cardiovascular health.
    Foundational to these results is a continuous remote care model. Via the Virta app, patients receive access to clinicians and health coaches who provide expert, individualized guidance on nutrition and behavioral change, monitor biomarkers, and de-prescribe medications as needed. This innovation is key for adherence and retention, even with lower-income and other hard-to-reach groups

    Today’s announcement offers hope to reverse the economic burden of diabetes as well, which costs

    Community Care Plan Participates for a Second Year in Local Channel 10’s Big Bus Express and Donates Bikes and Toys to Marine Corps Reserve Toys for Tots

    For the second consecutive year, Community Care Plan participated in Channel 10’s eight annual Big Bus Toy Express. On Thursday, December 9, Community Care Plan was the first stop for the Big Bus Toy Express and helped get the bus ready for its journey through South Florida to collect donations for underprivileged children, including decorating the bus. Community Care Plan employees donated hundreds of toys and gifts, including sports equipment, board games, craft kits, artwork sets, dolls, and much, much more from. The organization also donated 100 bikes from tricycles to larger bikes that covered various ages.

    Community Care Plan, the health plan with a heart, announced today that the organization and its employees donated 100 bikes and hundreds of toys to the Marine Corps Reserve Toys for Tots program.

The organization also donated 100 bikes from tricycles to larger bikes that covered various ages. Santa even made a surprise appearance to help celebrate the donations!

Community Care Plan will use CareSignal's platform to send accessible text-messages that will empower members to self-manage chronic conditions, communicate symptoms in real time, and connect to Community Care Plan's Concierge Care Coordination team for additional support. 

    Community Care Plan, the highly-rated health plan owned by Broward Health and Memorial Healthcare System, has partnered with CareSignal, a Deviceless Remote Patient Monitoring™ company, to increase Medicaid members’ access to care from the safety and convenience of home. Community Care Plan will use CareSignal’s platform to send accessible text-messages that will empower members to self-manage chronic conditions, communicate symptoms in real time, and connect to Community Care Plan’s Concierge Care Coordination team for additional support.

    With members at home during COVID-19, the Deviceless Remote Monitoring program will enable timely identification of members who have immediate health needs.     Community Care Plan served as a pickup location during the seventh annual Big Bus Toy Express held on Friday, December 11, 2020. Community Care Plan’s employees donated toys and gifts for children every year but given how difficult of a year it has been for many families, the donations were even more generous this year. The donations included hundreds of items, including sports equipment, board games, craft kits, artwork sets, dolls, and much, much more! The organization also donated 100 bikes that ranged from tricycles to larger bikes and covered all ages.

    “Community isn’t just a word in our name, it truly represents who we are and what we care about,” said Jessica Lerner, Community Care Plan’s President and Chief Executive Officer. “With so many families in need this year, we wanted to do something big and this event provided us with the opportunity to do that. Since so many children have had to stay inside most of the year, in addition to the employee donations, we thought that gifting bikes as an organization would help get kids outside and stay active, while socially distancing.”

    In addition to Community Care Plan’s donations, members of the public were invited to drop off their gifts outside the Community Care Plan office. Children of various ages, adults, and seniors also brought donations. With these donations, Community Care Plan and its employees helped to fill the second moving truck that was used to pick up the toys and bikes. Community Care Plan’s donations were so plentiful that they were also loaded onto the Big Bus Toy Express!

    With Broward County Public Schools starting in-person classes, Broward Health, Memorial Healthcare System, and Community Care Plan are pleased to announce that they will be holding their  2nd Annual Vaccinate Broward initiative to help ensure children in Broward County are fully vaccinated. The Vaccinate Broward event will provide free vaccinations to Broward County children and young adults, ages 4 to 20, on Saturday, October 24, from 9 a.m. – 1 p.m.  To adapt to COVID-19 precautions, Memorial Healthcare System will offer drive-up service at Memorial Primary Care in Hollywood, Florida. Broward Health will offer vaccinations at three of its locations, including Fort Lauderdale, Weston, and Pompano Beach.

    “We are excited to partner with Broward Health and Memorial Healthcare System to host the 2nd Annual Vaccinate Broward event,” said Jessica Lerner, President & CEO of Community Care Plan. “The ultimate goal of this event is to keep kids safe, healthy, and informed about the importance of getting vaccinated, which is especially important right now.”
     
    Broward Health, Memorial Healthcare System, and Community Care Plan launched Vaccinate Broward last year and are holding the program again to help educate the public about disease prevention through vaccinations, dispel common misconceptions about vaccinations, and provide free vaccinations. The Vaccinate Broward initiative provides families with greater access to care through alternative locations for vaccination. 

    “Our priority is the health, safety and well-being of our community and the patients we serve,” said Melida Akiti, Vice President, Ambulatory and Community Services, Memorial Primary Care. “Our collaboration with Vaccinate Broward is a way for us to move health forward and positively impact the health of our community from a population health perspective.”
     
    To protect patients, families and caregivers, each vaccination site has put additional safety measures in place, following CDC recommendations for COVID-19. Youth and parents must wear masks during the vaccination visits, and visitors will be socially distanced.

    In 1971, Family Central, Inc. (FCI) began as an organization to assist working poor families in need of subsidized childcare. He will also be responsible for ensuring that all operations are run in an efficient and effective manner, as well as creating an environment where both members and providers are able to obtain superior customer service. A seasoned health care executive, Ken has been working with government programs for over 20 years, delivering services to Medicare, Medicaid, and special needs populations. His specialty in performance improvement will enhance services in all areas. He holds a bachelor’s degree in Industrial and Systems Engineering from the Georgia Institute of Technology.
     
    As Senior Vice President & CFO, Jason Grynbaum will be responsible for directing the fiscal functions of the agency in accordance with generally accepted accounting principles (GAAP) and cost reimbursement principles relating to the health care industry and in keeping with the goals and objectives of the organization.  He brings 19 years of progressive leadership experience in corporate and operational finance within healthcare and manufacturing industries to CCP and has held various senior management roles within national managed care organizations. Prior to joining CCP, he led financial planning and analysis, reporting, medical economics and cash management for several large healthcare organizations and was responsible for driving business growth by offering differentiated value and excellent customer experiences. He earned his Master of Business Administration from Carnegie Mellon University and has a Bachelor of Science degree in Finance from Tulane University.

    Care Plan (CCP), the health plan with a heart, is pleased to announce that the National Committee for Quality Assurance (NCQA) has increased the accreditation status for CCP's Medicaid Plan from Accredited to Commendable. CCP's original Accredited status was awarded to CCP in December 2018 and the status increase is a result of NCQA's annual review of Healthcare Effectiveness Data and Information Set (HEDIS®) data for 2019. 

    Community Care Plan (CCP), the health plan with a heart, is pleased to announce that the National Committee for Quality Assurance (NCQA) has increased the accreditation status for CCP’s Medicaid Plan from Accredited to Commendable.     South-Florida-based Community Care Plan (CCP), the health plan with a heart, is pleased to announce that it was awarded a contract to provide subsidized Children’s Health Insurance Program (CHIP) and full-pay health insurance coverage through the Florida Healthy Kids plan, which serves school-aged children from age five through the end of age 18.  Effective January 1, 2020, CCP will provide coverage in eight Southeast and South Florida counties, including Broward, Indian River, Martin, Miami-Dade, Monroe, Okeechobee, Palm Beach, and St. Lucie.
     
    The Florida Healthy Kids subsidized plan currently serves nearly 193,000 children who are eligible for the subsidized CHIP plan based upon their family’s total household size and annual income, among other eligibility factors. Families who are not eligible for subsidized coverage may purchase coverage for their children through the Florida Healthy Kids full-pay plan, which currently serves approximately 15,500 children throughout the state and is competitively priced and child-centered compared to other options.

    Free Vaccinations for Children and Young Adults will be offered on Saturday, August 3, at Select Broward Health and Memorial Primary Care Locations in Broward County
     
    With the start of the 2019/2020 school year approaching, Broward Health, Memorial Healthcare System, and Community Care Plan are pleased to announce the launch of Vaccinate Broward, a collaboration to ensure that children in Broward County are fully vaccinated. Vaccinate Broward will educate the public about disease prevention through vaccination, dispel common misconceptions about vaccinations, and provide free vaccinations to Broward County children and young adults on Saturday, August 3, from 9 a.m. to 2 p.m.
     
    Free vaccinations will be available for 4- to 20-year-old children and young adults, including those attending college in the fall. Vaccines to be offered include:
     
    • DTaP: Diphtheria, Tetanus, Acellular Pertussis (Whooping Cough)
    • IPV: Inactivated Poliovirus
    • MMR: Measles-Mumps-Rubella
    • Varicella: Chickenpox
    • HPV: Human Papillomavirus Infection
    • Tdap: Tetanus, Diphtheria, Acellular Pertussis (Whooping Cough)
    • Meningococcal: Meningitis

To receive necessary vaccines, a parent or guardian must accompany all children under the age of 18 and bring the child’s immunization records and a valid form of identification.     Vaccinations are one of the best ways parents can protect infants, children, and teens from 16 harmful diseases. It is important that parents ensure their children are up to date on their vaccinations, particularly before the new school year begins.

    Community Care Plan (CCP), the health plan with a heart, is pleased to announce that the National Committee for Quality Assurance (NCQA) has awarded an accreditation status of Accredited for service and clinical quality to CCP’s Medicaid Plan. The Accreditation, awarded in December 2018, will be valid for three years.

    NCQA Health Plan Accreditation evaluates the quality of health care that health plans provide to their members. NCQA Health Plan Accreditation is a voluntary review process and evaluates how well a health plan manages all parts of its delivery system – physicians, hospitals, other providers, and administrative services in order to continuously improve the quality of care and services provided to its members. The NCQA Health Plan Accreditation is a nationally recognized evaluation that purchasers, regulators, and consumers can use to assess health plans.

    Community Care Plan (CCP), the health plan with a heart, is pleased to announce the promotion of Jessica Lerner to President and Chief Executive Officer of the company, as the former Chief Executive Officer, John A. Benz, announces his retirement. Lerner, who will take over the leadership role immediately, previously served as the company’s Executive Vice President and Chief Operating Officer. Benz will transition to the role of President Emeritus and Chief Strategic Officer of the organization.

    Benz and Lerner, who have worked together in the healthcare industry for over 23 years, will continue to work together to ensure a seamless transition. The two were part of the original leadership team who cofounded CCP. As President and CEO, Lerner will continue to grow CCP and provide quality-based healthcare to the South Florida community.

    “It is my privilege to turn over the role of President and CEO to Jessica as I continue to serve Community Care Plan in an advisory capacity,” said John Benz. “Jessica is an outstanding leader with tremendous vision, who I know will continue to grow Community Care Plan and lead the company to great success in the future.”

    Alongside Benz, Lerner co-founded CCP and played a major role in the operations and monitoring of local activities of the health plan to ensure that CCP is in accordance with contract requirements. She was promoted to Executive Vice President and Chief Operating Officer in 2016. Prior to joining CCP, Lerner served as Executive Director for Memorial Integrated Health for more than 19 years, where she spearheaded various initiatives including state contract negotiations, directing all health plan operations, inclusive of member benefits, provider network development, claims payment operations and system configuration, as well communications distribution and medical and pharmacy management. Lerner received a Bachelor’s Degree in marketing and management from Florida International University’s College of Business, and a Master’s of Healthcare Administration Degree from Florida Atlantic University. She also serves on the Board of Trustbridge Hospice Foundation and the Association of Community Affiliated Plans.

    After more than 43 years in the healthcare industry, Benz is now looking forward to transitioning to a new phase in his life and renewing his commitment to balancing his life. He takes great pride in the executive leadership roles that he has held throughout his career. He has worked diligently to strengthen the community by focusing on accelerated performance in service, quality, finance, people, and growth. Prior to leading CCP, Benz served as Senior Vice President and Chief Strategic Officer for Memorial Healthcare System (MHS). In this role, he provided executive leadership and propelled MHS as a leader in providing high-quality health care services to the South Florida community. As a result, MHS is now recognized as a national leader in quality health care and is highly regarded for its exceptional patient and family-centered care.
