Community Care Plan is the health plan with a heart, and you’ll notice that right from the start. We are owned by Broward Health and Memorial Healthcare System, and our mission is to provide healthier communities. That mission starts with providing your child with access to quality health care with our large network of pediatricians and specialists.

As a parent/guardian to a new member, we want to connect with you in the first month that your child joins our plan. We want to help you learn more about your child’s plan and benefits, find doctors or specialists, and get access to medical records.

As a CCP member, your child has access to our secure member portal called MyChart. This portal is available from your computer or your phone. With MyChart, you can:

• Request changes to your child’s Primary Care Provider (“PCP”)
• Order a new ID card
• Check your child’s test results
• Complete Health Questionnaires for your child
• Connect with our Member Services team

Get Rewarded for Completing and Returning the Health Risk Assessment (HRA)!

Did you know that you can get rewarded from CCP if you complete and turn in your child’s HRA? To help us better understand your child’s health needs, please fill out the HRA. This survey only takes a few minutes and will help our nurses find the best programs and services for your child. You can earn a gift card for completing all the information on the survey and sending it back to CCP within 90 days.
