1. Choose Your Primary Care Provider (PCP)
Login to your CCP Cares MyChart account.
Select the menu icon and in the Insurance section, pick “Choose my PCP”.
Search and choose any in-network provider. Submit the form.
If you can’t find your PCP or need help finding a doctor, call us at 866-899-4828.

We want members to get the best care across all areas of their health.

You can earn rewards for getting exams and tests that will help you and your family stay healthy, like:
Some rewards, like the Health Snapshot, are automatically given to you once you qualify, and others you have to ask for.

The CCP Prepaid Visa® card lets you spend rewards you’ve earned and some approved expanded benefits like fitness memberships, swimming lessons, medically related home care services, and legal guardianship (SMI members only).
The card is automatically mailed to the member's address. One card is issued to the responsible party, adult, or eldest person at the member’s address. The cards are not issued to minors.
Funds are only added or deposited to the account and available to use with the card when a reward or expanded benefit is approved.
