Our Case Management programs are free and give you extra help to take care of your health. If you have a serious health problem, are pregnant, or need help after a hospital stay, we have a team ready to support you.

Case Management means you get help from a nurse or social worker who works with you and your doctor. They help you:
- Set up doctor visits
- Learn about your health
- Get ready to leave the hospital
- Find services like food, housing, or medical equipment
Feel better and stay on track with your care
You can join or stop the program anytime. It’s your choice.

Programs We Offer

- Transplant Case Management – Help before and after an organ transplant
- Long-Term Care Transitions – Help when moving from a nursing home or hospital back to your home
- NICU Case Management – Support for parents with babies in the Neonatal Intensive Care Unit
- Medically Complex Pediatric Case Management – Help for children with serious or many health problems
- Complex Case Management – For people with many health problems. We help you manage your care and feel better. To join, you must meet certain criteria.
- Maternity Case Management – For pregnant members. We help you before and after your baby is born. We make sure you get the care you need.
- Care Coordination – Helps you connect all the care you need. If you see many doctors or take a lot of medicine, we help make sure everything works together. We also help with things like housing, food, or transportation.

- You are getting ready to leave the hospital
- Your doctor or our plan thinks you may need help
