Community Care Plan has programs to support members with chronic conditions

The goals of our Concierge Care Coordination (C3) programs like case and disease management are to support our members and caregivers with their care plan, get healthier, and improve their lives. Members and caregivers can learn more about their health and chronic conditions, and how you, or your loved ones can improve their health.

The chronic conditions that our Case Managers help with are:

Asthma

What is asthma?

  • Asthma is a chronic lung disease that makes it harder to move air in and out of your lungs.
  • Asthma can cause symptoms like wheezing, mucus production, coughing, shortness of breath, and tightness in your chest.
  • There is no cure for asthma, but it can be managed so you live a normal, healthy life.

How to take care of yourself

  • Get your influenza and pneumonia vaccinations
  • Know and stay away from asthma triggers
  • Watch your breathing
  • Follow up with your doctors
  • Quit smoking or don’t start
  • Know when you are having an attack and treat them early

  • Take medications, as directed

  • Diabetes is a group of diseases that affect how your body uses blood sugar (glucose) and causes your blood sugar to be too high.
  • Diabetes also affects how your body makes, or uses, insulin, and if your body doesn’t make enough, or doesn’t use it properly this causes your blood sugar to be high.
  • Diabetes, if not controlled, can hurt the eyes, kidneys, nerves, and heart.

What is HIV/AIDS?

  • Human Immunodeficiency Virus (HIV) is the virus that causes Acquired Immune Deficiency Syndrome (AIDS). The virus damages the body’s immune system and makes it easier for you to get sick.
  • HIV is generally spread through sex with an infected partner. It can also be spread through contact with infected blood or body fluids, like dirty needles.
  • Some people have no symptoms, but other people can have fever, chills, unexplained rashes, fatigue, night sweats, flu like symptoms, and mouth ulcers.
  • There is no cure for HIV, but the virus can be controlled with treatment. If the virus is not controlled, a person’s immune system will become weak to the point where they continue to AIDS and are at risk for serious infections and cancers because of their weak immune system.
  • AIDS symptoms are rapid weight loss, returning fevers, extreme fatigue, continued swelling of lymph nodes, diarrhea, and profuse night sweats.

 

How to take care of yourself

  • Prevention
    • Don’t have sex
    • Have safe sex using condoms the right way every time you have sex
    • Take PrEP (pre-exposure prophylaxis), a medication that people take to avoid HIV infection from sex and injection drug use. This needs to be taken as prescribed to be effective.
    • If you think you have been exposed, PEP (post-exposure prophylaxis) is a medication that people can take within 72 hours of exposure to avoid HIV
    • Don’t inject drugs. If you do, use new, clean needles and injection equipment every time. You can get new needles and syringes, and safely throw away used ones at a syringe services program (SSP). Locate a SSP near you or visit www.nasen.org/.
  • HIV Treatment and Self-Care
    • If you think that you may be at risk for HIV infection, get tested.
    • If you become HIV positive, your doctor will prescribe antiretroviral therapy (ART). This is a medication that usually needs to be taken every day. There are some treatments that can be taken as an injection every other month. Talk to your doctor about the treatment that is right for you.
    • Take your medicine, as directed, and do not miss any doses. This is important for you to stay healthy.
    • Get your blood test regularly, as directed by your doctor. This checks how your immune system is doing and helps make sure your treatment is working.
    • Stop smoking or don’t start.
    • Follow up with your doctors, as directed.
    • Keep a healthy diet
    • Get regular physical activity (as directed and approved by their doctor),
    • Manage stress
    • Have good sleep habits

What is hypertension?

  • Hypertension, also known as high blood pressure, is when the force of your blood against the walls of your blood vessels is too high.
  • Normal blood pressure is less than 120/80.
  • Blood pressure of 130/80 or more means you have a high blood pressure.
  • If your blood pressure is high for too long, it can hurt your heart, blood vessels, and other areas of your body, like your eyes and kidneys. It can also increase your chance of having a heart attack or stroke.

  • Ask your doctor questions to make sure you know how to take care of yourself
  • Watch and lower how much sodium (salt) you eat. The American Heart Association recommends no more than 1500 mg of sodium (salt) per day. Read food labels.
  • Keep a healthy weight. If you are overweight, losing weight can help you lower your blood pressure.
  • Watch how much alcohol you drink. Drinking too much alcohol can raise your blood pressure.

  • Get your regular preventive care like yearly checkup, regular blood pressure monitoring, and lab work

What is hypertension?

  • Hypertension, also known as high blood pressure, is when the force of your blood against the walls of your blood vessels is too high.
  • Normal blood pressure is less than 120/80.
  • Blood pressure of 130/80 or more means you have a high blood pressure.
  • If your blood pressure is high for too long, it can hurt your heart, blood vessels, and other areas of your body, like your eyes and kidneys. It can also increase your chance of having a heart attack or stroke.

  • People with Sickle Cell Disease are born with the condition that makes red blood cells become sickle, or crescent shaped.
  • The shape of red blood cells does not allow it to move easily through the body and can block blood flow. This leads to symptoms and serious health problems.
  • If blood flow is blocked, it can lead to strokes, eye problems, infections, blockages in the blood vessels of the lungs, severe pain attacks, and many other health problems.

How to take care of yourself

  • Ask your doctor questions to ensure you know what you need to do to take care of yourself.
  • Stay compliant with regular preventative care such as your annual checkup, vision exams, pulmonary function testing, echocardiograms, regular blood pressure monitoring, and lab work to monitor your health.
