Our Preferred Provider Program
At Community Care Plan (CCP), we believe outstanding care deserves to be recognized and rewarded. Our Preferred Provider program highlights and supports providers who consistently deliver evidence-based care and achieve exceptional results.
This initiative is designed to spotlight excellence, encourage continuous improvement, and strengthen the overall quality of the Statewide Medicaid Managed Care (SMMC) program.
What is a Preferred Provider?
When you see the CCP Preferred Provider badge at your doctor’s office or in our directory, you know you’re getting extra special care. These doctors and clinics are part of a special CCP program that makes sure you get high-quality care to help people like you get and stay healthy.
Criteria for High Performance
Each year, CCP evaluates providers using a rigorous and transparent review process. We assess performance across key domains, including clinical quality, access, regulatory compliance, and efficient resource use.
Key Performance Indicators (KPIs)
Quality of Care
- Consistent adherence to clinical guidelines and evidence-based practices
- High rates of preventive care (e.g., screenings, immunizations)
- Strong performance in managing chronic conditions (e.g., diabetes, hypertension)
Access
- Positive results on Potentially Preventable Admissions (PPA) and Potentially Preventable Visits (PPV)
- Timely appointment availability and reduced patient wait times
Cost-effective and efficient use of clinical resources
Utilization
- Low hospital readmission rates and reduced emergency department (ED) visits
- Strong performance in ED visits and inpatient admission metrics
- Effective coordination and continuity of care across services
