Welcome to the Before Baby & Beyond program! We want to make sure that you get the care and support that you need for a healthy pregnancy — before, during, and after your baby is born.
Our dedicated team can help you with everything from finding a doctor and birth planning to getting baby supplies and connecting with local experts for prenatal and postpartum care. Below are the resources we offer to guide you through your journey.

Health Snapshot
Get the most from your health plan benefits by completing our Health Snapshot. Your answers will help your or your child’s primary care provider (PCP) and our nurses know what you or your child need.

Your pregnancy is an exciting time, but it’s okay to feel a little nervous! Our Before Baby & Beyond program offers support from nurses, social workers, and health care professionals to help you at every step after your baby arrives!
Tell us you are pregnant (complete the pregnancy notification form) within 30 days of your doctor confirming your pregnancy and get $25 on our CCP Prepaid Visa.
How to Take Care of Yourself:
- Choose a doctor and know your options.
- Follow up with your doctor:
- First visit: As soon as you know you’re pregnant
- 4-28 weeks: Monthly visits
- 28-36 weeks: Biweekly visits
- 36-40 weeks: Weekly visits
- Postpartum: 7-84 days after delivery
- Get all your prenatal tests and screenings.
- Share your birth choices with your doctor.
- Watch for warning signs like headaches, blurred vision, shortness of breath, chest pain, dizziness, or fever. Call your doctor right away if you have any of these.

Congratulations on your new arrival! Whether you’re a first-time mom or expanding your family, we’re here to support your postpartum journey.
How to Take Care of Yourself:
- Schedule your postpartum visit within 7-84 days after delivery.
- Make an appointment for your baby’s first checkup.
- Ask your doctor for breastfeeding support and talk to them about how you’re feeling emotionally.
- Don’t skip postpartum checkups. Watch for warning signs like heavy bleeding, chest pain, or headaches. Contact your doctor right away if you have any of these.

- Always put your baby to sleep on his or her back for naps and at night.
- Keep the sleep area clear of any loose items like blankets or toys.
- Nurse or feed your baby when he or she shows signs of hunger — most babies eat every two to three hours.
- Our care managers are here to help — reach out anytime for support!
- Lactation consultants are available if you have questions about breastfeeding or feeding.
- Stay in touch with your baby’s doctor for regular checkups.
- And remember, it’s OK to ask for help when you need it!
