Get Rewards for Taking Care of Yourself and Your Family!

Community Care Plan rewards members for taking steps to get and stay healthy. You and your family can earn rewards on your CCP Prepaid Visa for getting health exams, completing programs, and more.

Some rewards, like the Health Snapshot, are automatically given to you once you qualify, and others you have to ask for.

MEMBER SERVICE OR PROGRAM DESCRIPTION REWARD
All Health Snapshot Complete CCP’s Health Snapshot within 45 days of enrollment effective date (listed on the member ID card) or CCP’s request date. An additional $5 reward will be given if a member or parent/guardian completes the Health Snapshot online. This reward is automatically added to the CCP Prepaid Visa after the Health Snapshot is received. $15 or $20
Pregnant Tell Us You Are Pregnant Tell us you are pregnant within 30 days of your doctor confirming your pregnancy $25
Pregnant Health Snapshot Complete CCP’s Health Snapshot within 60 days of CCP’s request date. This reward is automatically added to the CCP Prepaid Visa after the Health Snapshot is received. $25
Pregnant Prenatal Visit(s)
  • First Visit – Complete your first prenatal visit with your doctor within your first trimester or first three months of each pregnancy
  • During pregnancy – Complete prenatal visits with your doctor throughout each pregnancy
  • $50

  • $25 each visit, up to $100 for each pregnancy
Pregnant Postpartum Visit Complete a postpartum visit with your doctor within seven (7) to 84 days of delivery $25
Newborn First Check-up Complete one (1) checkup by a doctor within 30 days of birth $25
Ages 2 and up Yearly Check-up or Annual Wellness Visit Complete one (1) check-up per year with your Primary Care Provider (PCP) or OB/GYN $20
Ages 6 and up Behavioral Health Follow-Up Complete one (1) follow-up visit with a behavioral health provider within seven (7) days after a hospital discharge for a behavioral health diagnosis $25, up to $50 each calendar year
Any Member with a Diabetes Diagnosis Diabetes Management A1c blood test with a result of less than 8 $20 each calendar year
Ages 12 and up Substance-Use Disorder Program Complete a Substance-Use Disorder program with:
  • An initial psychiatric assessment, and
  • Two total follow-up visits
    • One (1) follow-up visit within 30 days, and
    • One (1) follow-up visit within 90 days

Ages 12 and up Tobacco Cessation Program Complete a tobacco cessation program with proof of completion from Area Health Education Centers (AHEC) or Tobacco Free Florida ** $50
Ages 5 and up, with a BMI of 40 or 95th percentile for a child Obesity Management Program Complete a six (6) month Obesity Management program with:
  • Two (2) PCP visits, and
  • Three (3) visits with a Registered Dietitian in combination with Care Management
 $50

To Get Your Reward

  • You must be a CCP member when the service or program was completed. If you leave our plan for more than 180 days, you may not receive your reward.
  • You have six (6) months from the time the service or program was completed to complete the reward form.
  • Please use the Rewards Form to request a reward. One form for each family member. For doctor visits, please enter the date that each visit was completed.
  • – Health Snapshot: You do NOT have to request your reward for completing the Health Snapshot. The reward will be added to your Community Care Plan Prepaid Visa® once approved.
  • – Tobacco Cessation Program: Please email the member’s name, member ID #, and cell phone # with the proof of completion from the AHEC or Tobacco Free Florida Cessation Counselor/Instructor to ccpreward@ccpcares.org.
  • Rewards can not be transferred.
  • Rewards may not be used for gambling, alcohol, tobacco, or drugs (except for over-the-counter drugs).

Rewards Added to the CCP Prepaid Visa®

All members are sent a Community Care Plan Prepaid Visa®.

CCP will add your reward funds to the Community Care Plan Prepaid Visa®.

The card gives you an easy way to pay for rewards that you’ve earned and some of our expanded benefits.

Your prepaid Visa is ACTIVE starting on your plan effective date. Be sure to sign the back of your card.

Funds are only deposited in your account and available to use with your card when your reward or expanded benefit is approved.
